I am from a doctors office and I need a copy of a Physician
Certification Statement (7KB PDF file). You can download the statement
and return it to us.
1. I have insurance, can you
bill them directly?
We do submit claims to several insurance companies. If you would like
to have us submit your claim, please fill out the yellow form that
came with your first invoice and return it to us. You
may also download this form here, if you would like. If you received
an informational invoice from us, we have submitted your claim for
you, and your insurance information should be displayed on the bottom
of the invoice.
2. I was in a car accident
- how is that billed?
If you were transported as a result of a motor vehicle crash, your
medical expenses are covered under the No-Fault portion of your vehicle’s
insurance. After notifying your insurance agent, please contact our
office with your insurance information and claim number so that we
may submit your claim or you can fill out our Insurance
& Signature form (6kb) pdf file and return it to us by mail
or fax. New York Law requires that the claim be submitted to the insurance
carrier within 45 days of the accident. Please be prompt in providing
us your No Fault Information!
3. I was injured on the job
and I received a bill, why is that?
Please ask your employer to contact our office immediately regarding
this. We need to have your workers compensation carrier information.
4. I have Medicare, do you
submit to them?
Yes, by law, we have to submit your claim to Medicare. There are many
steps to this process which if not handled immediately will result
in a delay of receiving your Medicare payment. We need to have a Signature
Authorization signed by either the patient or a responsible party,
our ambulance crews will try to do this while with you, if it is convenient
at the time. We also need to have a physicians certification statement
signed by the doctor that ordered the transport or that saw the patient
in the emergency room. Once we have this information, your claim will
be submitted electronically.
5. I have Medicaid and I received
a bill?
Please contact our office immediately with your identification number.
We will be happy to bill Medicaid for you.
6. I received a bill for a
"Medical Assist", will my insurance cover this?
In many cases, "Assist" charges are not covered by insurance
companies. If you have a question about an assist charge, your first
phone call should be to your insurance company to see if they cover
it. If they do cover it and you need assistance in submitting it,
please contact our office, we would be happy to do so.
7. Do you provide a wheel
chair transport service?
Yes, we do. We currently have three wheel chair vans and we can use
them to take a patient to a doctors appointment, home from the hospital,
from a nursing home to a residence, etc. In order to be a "candidate"
for a wheel chair transport, the person must not have any medical
conditions that require them to be constantly monitored, they must
be able to sit unattended in their wheel chair, they also need to
be alert to their surroundings.
8. I received a bill for a
"Fly Car Service", what is that all about?
Volunteer ambulance services during times when they have difficulty
staffing their ambulance (example during the weekdays when most volunteers
are at work) and also if they need additional resources or medical
assistance. You can view more of the details about the Fly Car Service
by clicking here. If you received a bill from Bangs Ambulance for
this service, we were requested by the transporting ambulance service
or the Tompkins County 911 center.
9. Why do I get both invoices and statements?
For every service that we provide, you will receive one invoice with detailed information on it, including if we have billed your insurance company. Once a month you will also receive a statement reflecting any outstanding balances that are considered to be your responsibility..
If you have any questions, please call us or E-mail us and we will
respond as soon as possible. Thank You!
